Join our Innovative Contessa Health team as a Nocturnal Triage RN, supporting acute care in the home for patients across multiple hospital systems. This is a remote position, where you will serve as a liaison between patients and physicians, providing virtual care and handling escalations to ensure patients receive top-tier healthcare overnight. Your expertise in virtual triaging, medication administration, and patient management will be vital in ensuring smooth care transitions and optimal patient outcomes.

Responsibilities
- Act as the liaison between patients and physicians, providing timely updates on patient status and escalations.
- Virtually triage incoming patient calls, managing escalations and addressing concerns in real time.
- Administer medications virtually, ensuring accurate dosages and prompt delivery.
- Take and document verbal reports from field RNs to ensure continuity of care.
- Address and manage virtual vital sign alerts, escalating as necessary to physicians or care teams.
- Conduct virtual patient calls for health assessments, medication guidance, and patient care follow-ups.
- Document patient interactions and care interventions in both hospital EMRs and Contessa’s proprietary EMR system.
- Maintain active RN licensure across all relevant markets to support multi-state coverage and compliance
Qualifications
- Associate of Science in Nursing.
- Current, active RN licensure in good standing and willing to be licensed in multiple states at Contessa's expense.
- Three to five years of acute nursing experience.
- Possess a general knowledge of clinical standards and outcome measurement.
- Must have reliable transportation and proof of liability insurance
- Ability to support after-hours schedules and on-call needs.
