Description
Under supervision of a Physical Therapist and in accordance with agency policies, gives medically prescribed therapy. Provides advisory services to other professional workers who share responsibility for client care. Participates in the development of the physical therapy program. Functions require special skills and knowledge of public health principles and practices. Each client visit will be documented on the appropriate clinical or progress note.
Performs physical therapy procedures on clients for whom these have been medically prescribed
Teaches appropriate treatment procedures and supportive activities to client and family members as delegated and supervised by the therapist.
Translates all exercises into functional activities or activities of daily living.
Assist with developing needed plan for modifying equipment, appliances and the physical surroundings in client’s home.
Confers regularly with the attending Physical Therapist regarding client’s progress. Reports to them the physical and emotional condition and reactions to treatment and interprets social and physical factors in the environment that affect client care.
Interprets to the clients and family the implications of the treatment consistent with the actions and wishes of the physician.
Helps the family accept responsibility for providing care.
Provides families with information, support and encouragement to help motivate them in their progress.
Helps individuals and families to accept and adjust positively to physical, mental and social limitations.Teaches basic principles of good posture and body mechanics in relation to changing needs of individual in all age groups.
Maintains necessary records. Prepares clinical and progress notes.
Provides advisory and consultation services to staff.
Demonstrates rehabilitation procedures and supportive activities on selected clients to nurses responsible for follow-up services.
Participates in the PI program by utilizing suggestion boxes, customer service, and brings matters to attention of management staff.
Maintains all education required by licensure is current. Complies with OSHA, CHAP, State and Federal Standards.
Demonstrates the ability to be flexible with workflow, department needs and work hours.
Updates management regarding potential problems or concerns. Also responsible for consulting with clinicians or managers as needed.
Maintains an understanding of the company’s scope of services.
Understands the infrastructure and how and where to transfer calls in the company.
Maintains a professional image.
Uses appropriate phone etiquette.
Participates in interdisciplinary conferences as required.
Submits clinical documentation in a timely manner according to agency policy.
Promotes a customer friendly atmosphere for all visitors and ensures client confidentiality at all times.
Participates in accreditation program.
Performs other duties as assigned.
