Position Summary:
The position is responsible for liaison and communication with Covenant Care referral sources, both individual practitioners and facilities. They will use a variety of resources to promote referrals to Covenant Care. Their goal is to ensure that all persons with a need for home health, either through the Inpatient Units or at home, choose Covenant Care.
Qualifications:
A minimum of 2 years of clinical nursing experience preferred. Experience must be in Home Health or in a medical/surgical section, a hospital, oncology or geriatric facility. A current RN license in the State of Florida or Alabama, as applicable depending on assignment. Reliable transportation, valid State of Florida or Alabama driver’s license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required. The applicant must have strong communication skills.

Main Duties and Responsibilities:
The primary responsibility of the Clinical Navigator is to assist key hospital staff in identifying patients appropriate for home health care, assess new referrals for appropriateness, provide recommendations for alternatives for care if the patient is not eligible for Home health services, provide education to key hospital staff, patients and families on home health care, admit appropriate patients and ease the transition between the hospital setting and the next location of care. Specific methods for reaching this overall goal are:
- Perform initial consultation visit to explain covenant services and establish appropriateness for Home Health.
- Conduct initial physical assessment of patient for the purpose of identification of immediate problems and follow-up as necessary.
- Obtain medical records to support admission to home health.
- Document thorough initial assess on the current status of the patient/family unit.
- Provide information regarding other community and healthcare services available if patient is not appropriate for home health.
- Communicate to patient/family a thorough introduction to Home Health philosophy and services available. When applicable, obtain Consent for Care and appropriate election states as required by payor source.
- Determines the patient is home health appropriate, acceptance and/or further guidance related to qualifications for admissions.
- Responsible for generating referrals to Covenant Care.
- Serve as an informed home healthexpert and patient advocate.
- Develops ongoing relationships with and provides education for key personnel in referral source offices or facility settings.
- Proficiency in Matrixcare EMR and various hospital referral platforms.
- Demonstrate knowledge in laws and regulations related to marketing and sales in home health and follow all policies and procedures of Covenant Care regarding inducement.
- Achieve performance goals both quarterly and annually.
- Collaborate with Home Health Operations Departments to assist them in achieving admission goals.
- Communicates to other team members the current status of patient/families in the IPC or ones coming to the IPC
- Participate in continuing education to promote professional growth. Stays updated on medical news, changes in the healthcare communities and maintain licensure, if applicable.
- Stays abreast of rules and regulations guiding Home health care in facilities.
- Informs Supervisor of changes/updates in facilities.
- Provides education for physicians, social workers and other sources that may need to refer patients for home health and services. Serve as a consultant to facilities regarding Covenant Care programs.
- Participate in weekly Sales Ops Meetings and Daily Huddle Teams calls.
- Promotes a positive image of the Home Health concept throughout all referral sources and in the community.
- Perform other duties as assigned by Regional Director of Business Development.
- Required to comply with all Covenant policies, procedures and personnel policies, including those specifically addressing disaster preparedness and response.
Special Demands / Physical Requirements:
This position may sometimes require light to medium lifting. The position requires some time traveling in a motor vehicle and some weekend and evening work in connection with performing essential functions of the position.
